The focus of the program is to
help each client realize their optimal level of functioning, good
health, independence and well-being. This is accomplished by a
well-coordinated interdisciplinary team of professionals who assess
each client and develop an individualized plan of care for them. A
network of program components is organized by the Program Director to
provide a continuum of care for each client consistent with their
